Physical therapy can help decrease a patient’s pain, improve his/her mobility and assist the patient with maintaining as active a lifestyle as possible. At Franciscan St. Francis Health, we provide specialized physical therapy to help patients improve their mobility and functionality and ultimately their quality of life.
Each physical therapy patient at Franciscan St. Francis Health receives a complete, detailed evaluation by a licensed physical therapist. All of our physical therapists have degrees in physical therapy at the bachelor’s, masters or doctoral level and are licensed by the state of Indiana.

Getting started
In Indiana, before you have any physical therapy services, you must have a physician’s written order from within the last 30 days. Once you have a physician’s order, you can call Franciscan St. Francis Physical Therapy to schedule an appointment.
You are responsible for knowing what is covered by your insurance prior to the start of therapy. As a courtesy, we will research your coverage to the best of our ability, and inform you of your physical therapy benefits.
Preparing for Your First Appointment
For your initial evaluation, please arrive 15 minutes early to complete the registration process and your paperwork. Please bring any written physician’s orders with you. For any physical therapy services in Indiana, you must have a physician order written within the last 30 days.

At your appointment, you will be evaluated by a physical therapist. Please make sure you:
- Wear appropriate attire, e.g., shorts and a T-shirt. You may need to expose the part that is injured.
- Bring any assistive devices, e.g., braces, to help with your care.
If you are not present at the start of your appointment time, you may have to reschedule. If you need to cancel your appointment, you need to provide 48 hours notice.
Checking your insurance coverage
You are responsible for knowing what is covered by your insurance prior to the start of therapy. As a courtesy, we will research your coverage, to the best of our ability, and inform you of your physical therapy benefits. However, because there are frequent changes in individual benefits, it is ultimately your responsibility to stay informed as to what those benefits are.
If you have questions, please check the back of your insurance card and contact your insurance company directly for coverage questions. Co-payments are due at the time of service.
